

The consultation is about reviewing the role of midwives in Indonesia’s primary health care transformation agenda.
As part of Indonesia’s 2020–2024 National Medium-Term Development Plan (RPJMN) and the Ministry of Health’s 2021–2024 Health System Transformation, this study supported the government’s efforts to revitalize primary health services, including expanding Posyandu Prima — community-based health posts offering promotive and preventive care at the village and sub-village levels.
The assignment involved conducting a situation analysis of health workforce resources, particularly midwives, within the evolving tiered health system. Through a combination of desk reviews and stakeholder focus group discussions, the study assessed policy alignment, scope of practice overlaps, and the adequacy of legal frameworks governing midwifery roles in the new service model.
The consultation identified critical gaps, including inconsistencies in midwifery authority regulations, unclear task distribution in inter-professional collaboration, and potential legal risks for midwives asked to perform services beyond their formal competencies. The study produced recommendations to guide policy adjustments, workload analysis, legal protection frameworks, and capacity-building strategies — ensuring midwives are effectively and safely integrated into Indonesia’s health system transformation.
